RN- VISITING NURSE, PER DIEM -HOME HEALTH job at Eden Home Healthcare, Inc. Glendora, CA, US

Job Description

Eden HHC is looking for experienced Registered Nurses to join our dedicated team. The candidates must have minimum of 1 year professional Home Health nursing experience. Our service area includes Los Angeles County, Orange County, Ventura County, San Bernardino County, Riverside County and Kern County. We use Wellsky/Kinnser for EMR. Position is for Per Diem to cover Glendora and surrounding areas. Candidate will set preferred areas to cover. Candidates must have:

Current active RN license in the state of California
Current CPR, Health clearance, TB clearance
Minimum of one year of professional Home Health nursing experience
Must pass criminal background check
Driver’s license
Proficiency in EMR charting and timely submission of documents
Proficiency in wound care, catheter insertion, tracheostomy care, diabetic care, vital signs monitoring, ostomy care

Responsibilities:

Complete on-going comprehensive assessments of the patient’s needs
Provide service and /or treatments requiring substantial and specialized nursing skills
Provide skilled nursing care under the direction of the RN or the physician
Educate and support the patient, family and caregiver in meeting nursing and related needs
Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance.
Implements the patient's individualized plan of care and participates in case conferences
