Job Description

*Registered Nurse (RN) For Assertive Community Treatment Program*

Interborough Developmental and Consultation Center, Inc a licensed out-patient behavioral; health organization that is committed to providing the highest level of care to the Brooklyn community is seeking a Registered Nurse (RN) for its Adult ACT Team.

Assertive Community Treatment (ACT) is a community-based and mobile team intervention designed for people with behavioral health challenges and at risk for hospitalization, involvement with the justice system and homelessness. ACT in New York uses the transitional care framework to structure the work of the team:
* Engaging & envisioning life goals;
* Engaging in wellness self-management; and
* Integrating with the community

Job Type: Full-Time

Salary: $90,000-$102,000 (based on experience and licensing)

*QUALIFICATIONS:*
* Registered Nurse (RN) with current New York State license
* At least three years of experience working with adults (ages 18 and up) in one or more of the following areas: psychiatric nursing, substance use treatment setting, social service setting(s) caring for individuals experiencing homelessness, justice involvement or limited social support;
* Extensive knowledge of community treatment, support services, and resources
* Highly organized and self-motivated
* Spanish/Creole Speaking preferred
* Proficient in computers
* Travel or Special Conditions/Requirements: Traveling required.
* Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
* Ability to prioritize tasks and manage time effectively in a fast-paced environment.
* Attention to detail and strong problem-solving skills.
* Commitment to maintaining patient confidentiality and privacy.

*JOB DESCRIPTION:*
* Provides at least 50% of health services to consumers in the community, including but not limited to: medications administration (both oral and intramuscular), monitoring medications side effects and adherence, phlebotomy, specimen collection, and physical assessments;
* Coordinates consumers’ health care services, providing psychoeducation to consumers about health and medications, assessing consumers’ mental statuses;
* Coordinates with prescriber on consumers’ medication management needs, health care services, maintaining consumers’ medication administration schedules;
* Coordinates care with family and other community service providers or collaterals including, but not limited to: hospitals, housing providers, shelter staff, criminal justice personnel, Assisted Outpatient Treatment (AOT), and primary health care providers;
* Coordinates to provide in person and telehealth walk in services;
* Completes and signs progress notes, medical assessments, service plans, substance and risk assessments, discharge plans, incident reports, and other required documentation in accordance to the New York State Office of Mental Health (OMH) regulations, the New York City Department of Health, Mental Hygiene (DOHMH) guidelines, Medicaid billing standards and regulations and IDCC policy and procedure;
* Participates in team meetings, clinical meetings, and nurses’ meetings to collaborate and discuss clinical issues and clients’ needs with team members;
* Uses motivational interviewing, culturally and developmentally appropriate, recovery oriented and trauma-informed approaches when delivering services;
* Provides health awareness and information to community members in outreach events and group setting.
