The House Calls Medical Assistant/Scribe (CIS/MA) supports logistics, clinical care, and documentation in Oak Street Health's innovative home-based care program. The CIS/MA works directly with a House Calls Provider daily to prepare patient visits, document visits in the EMR accurately, identify and bridge gaps in care, and advocate for patients and families. Responsibilities include preparing daily visit schedules, accompanying providers on home visits, checking vital signs, administering vaccinations, performing point of care tests, blood draws, assisting in medication reconciliation, supporting patient and family education, documenting patient encounters with history, assessment, plan, physical exam, assigning CPT and ICD-10 codes, preparing after visit summaries, consulting with providers for accurate documentation, improving clinical documentation, requesting and reviewing medical records, leveraging population health tools, supporting daily huddles and clinical documentation reviews, administrative support, managing orders and referrals, maintaining supplies and equipment, participating in team meetings, and other duties as assigned. Required qualifications include Medical Assistant certification, CPR or BLS certification, minimum 1 year experience, clinical competencies, vehicle access with valid license and insurance, flexibility to travel, EMR experience, computer skills, and US work authorization. Bilingual (English and Spanish) is a plus.
