Community Bridges, founded in 1982, is a non-profit agency with a mission to advance the integration, growth and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access and find creative solutions to develop meaningful community engagement.

We are currently looking for 2 part-time Life Skills Coaches for the Community Support Services Department in Concord, NH !

The Life Skills Coaches provide coaching and assistance to adults with intellectual disabilities. This includes assisting with skill development on activities of daily living which may include supports with money management and budgeting, meal planning and preparation, health maintenance and expanding knowledge of community resources. Some other activities could include assisting with shopping; coaching to improve living conditions or practicing good communication and social skills. The individuals do not live in a Residential Home and live in their own homes. The Life Skills Coach takes the goals that the individual is trying to achieve and help them develop the skills over time.

Qualifications:

Education: High School Diploma or GED required.

Experience: Experience in the human service or related field with persons experiencing disabilities desirable.

Reliable transportation, a valid driver's license, and proof of automobile insurance.

Compensation:

There is a $1000 sign on bonus for this position - $500 will be paid at time of hire and $500 will be paid after 6 months in the role.

Paid Time Off: For a full-time employee working 40 hours a week, 264.16 hours of Paid Time Off is received per year! PTO days will increase over the years based on longevity and there is no waiting period to start using your PTO!

Mileage Reimbursement: Employees are reimbursed for approved mileage driven on the clock.

Paid Training: As an employee, you are paid your usual rate of pay while attending trainings!

Current Open Positions Available:

2 Part Time: Monday through Friday (20-29 hours)

Specific Duties and Responsibilities:

• Performs all duties in a manner that demonstrates an awareness of and respect for, each person's legal, civil, and human rights; including maintaining confidentiality of information regarding the individual that ensures the respect and dignity of persons with a developmental disability.
• Assist individuals in establishing, maintaining, and presenting a positive image.
• Assist each individual to identify places, activities and resources in their own community.
• Assist in developing the individual's goals by providing information for assessments and by developing strategies for goals and methods for learning.
• Complete documentation requirements as specified by the Individual Service Plan to include weekly progress notes, incident reports, and medical office visit reports and meet specific deadlines for this documentation.
• Attend team meetings, quarterlies and other agency meetings for the individual as assigned.
• Attend trainings as required. Meet and maintain all mandatory training requirements as a condition of employment.
• Report any safety, health or human rights concern immediately to the Supervisor.
• Follow all applicable program policies and procedure of Community Choices at all times while representing Community Choices and the individuals we support.
• Understand and ensure compliance with applicable state regulations.
• Submit Mileage Reports and Timesheets within agency's timeframe.
• Perform other duties and responsibilities as assigned.
