Job Description

Duties
The Social Worker is a key member of the interdisciplinary team, responsible for assessing psychosocial symptoms of patients and families, developing outcomes, and providing psychoeducation, counseling, and casework interventions. Key duties include:
Conducting bio-psycho-social-spiritual assessments.
Offering individual and family supportive counseling.
Referring to community resources.
Maintaining accurate patient documentation.
Participating in care planning and team meetings.
Providing bereavement care and psychosocial education.
Qualifications include a Master's in Social Work, at least one year of healthcare experience, and relevant state licensure. Strong communication skills and the ability to work in a team are essential.
