Job Title: RN Home Health Case Manager

Location: Denver Metro Area, Colorado

Salary: Starting at $42 per visit

Schedule: Flexible schedule, Monday through Friday

Job Description:

We are currently seeking a Registered Nurse (RN) to join our team as a Home Health Case Manager in the Denver Metro Area. As a Case Manager, you will be responsible for providing compassionate and comprehensive home health care services to our patients.

Responsibilities:

Conduct initial patient assessments to develop personalized care plans.
Provide skilled nursing care to patients in their homes.
Monitor and evaluate patient progress, adjusting care plans as necessary.
Coordinate with other healthcare professionals and community resources to ensure continuity of care.
Educate patients and their families about their conditions and treatment plans.
Document all patient care activities accurately and in a timely manner.

Qualifications:

Current Colorado RN license.
Minimum of 1 year of experience in home health or a related field.
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal skills.
Ability to work independently and manage time effectively.
Valid driver's license and reliable transportation.
