CleanSlate Centers is a nationwide medical group providing office-based outpatient treatment for the Americas crisis of addiction. Our Mission is to improve the quality of life for people suffering from opioid and alcohol addictions. We're accomplishing this by providing high-quality medication-assisted treatment (MAT) led by physicians and intensive care coordination with a wide network of community partners. We are currently looking for a Nurse Practitioner for our outpatient substance abuse treatment center located in Lafayette, IN! This position entails the initiation and maintenance of medication assisted treatment primarily, but not exclusively for opioid and alcohol dependence.

Qualifications:

Must be a licensed healthcare professional NP or PA with an active DEA license. Demonstrated ability for interpersonal and communication skills.
Superior organizational skills. EMR experience and computer fluency.
Professionalism in all communications internally and externally. Team player with high adaptability. Ability to improve with feedback and constructive criticism.
Limited travel is possible.
