Position Summary:
The Community Participation Support (CPS) Workers provide assistance and opportunities for individuals with intellectual disabilities, mental health, and behavioral health needs to engage with their community, develop life skills, pursue interests, meaningful social connections and increase independence.

·         Follow CPS plans, schedules and instructions created by the CPS Specialist

·         Support individuals in working on simple skill goals such as social interaction, communication, community safety,            money skills, and decision-making.

·         Encourage independence while offering appropriate prompts and support.

Community Outings

·         Take individuals into the community based on their schedules and interests.

·         Assist with accessing community locations such as stores, parks, libraries, restaurants, gyms, and other approved            sites.

·         Engage with individuals during activities — no passive supervision.

·         Support participants in selecting appropriate, meaningful activities.

Safety & Supervision

·         Ensure individuals are supervised at all times during community activities.

·         Follow all safety plans, behavioral plans, and medical guidelines as trained.

·         Maintain appropriate ratios and report any ratio concerns immediately.

·         Monitor for changes in behavior, mood, or health and inform the CPS Specialist.

Documentation

·         Complete brief CPS notes at the end of each outing using the trained format.

·         Document where the individual went, what they did, goals they worked on, progress or concerns.

·         Report any incidents or unusual events immediately.

Transportation

·         Use CPS-provided agency vehicles to transport individuals.

·         Assist participants with safely navigating the community and following community rules.
