The Certified Nursing Assistant (CNA) in Transitional Care provides routine patient care tasks such as vital signs monitoring, phlebotomy, and basic patient transportation under the supervision of a Charge Nurse. The role requires California CNA certification, BLS certification, and experience in skilled nursing or transitional care settings. This full-time night shift position focuses on delivering compassionate care to patients ranging from young adults to geriatrics within a hospital environment.
Description
The Certified Nursing Assistant (Sub-Acute/Transitional) role is a non-licensed health care employee who performs delegated routine and predictable patient care tasks/duties and works under the direct supervision of a Charge Nurse. These duties may include, but are not limited to phlebotomy, EKG's, vital signs, monitoring, basic patient care and transportation. Responds appropriately to patients and families with knowledge of age specific needs based on scope of service of unit/department where assigned (Young Adult through Geriatric).
Supervisory Authority: None, however, expected to recommend actions to Lead CNA and Charge Nurse.
