As a member of a multidisciplinary health care team, the clinical pharmacist provides consistent clinical, consultative, research, and/or administrative pharmaceutical services for critical care, oncology/hematology, other advanced practice areas or participate as part of the rounding team for acute care areas. Clinical Pharmacists II select, compound, dispense, drugs, medicines, and other therapeutic agents; may serve as members and consultants on treatment or diagnostic teams; consult with nursing staff and physicians regarding medication orders; disseminate drug information to members of the hospital staff and interested community groups; may participate in hospital research projects or perform pharmaceutical research; provide recommendations to Clinical Research Pharmacists and Pharmacy management regarding new drug requests and non-formulary applications.
Identifies differences between drugs as they relate to therapeutic efficacy of the products for all areas including critical care and/or oncology/hematology.
Applies knowledge of the physical and chemical properties and characteristics of computing dosages, weighing and measuring drugs, and mixing and compounding drugs and chemicals for all areas including critical care and/or oncology/hematology.
Applies knowledge of solubility behavior of substances, stability of drugs, usages of drugs, and limitations on modes of administering drugs, and similarly applies knowledge of incompatibilities in evaluating dosages and permissible concentrations of drugs, and in anticipating, preventing, and treating undesirable chemical reactions for all areas including critical care and/or oncology/hematology.
Applies knowledge of pharmacodynamics and pharmacokinetics.
Consistently work in an advanced practice area as assigned by pharmacy leadership to promote and facilitate the adoption and assimilation of best practices and innovations in service, work methods, technology, and operational systems which improve pharmacy service, quality, and productivity.
May assist with discharge medication coordination, which may include assistance with prior authorizations and provide discharge medication education to patients.
May round with interdisciplinary team in acute care areas to provide drug information, coordinate medications, and provide drug education to patients and families on a rotational basis.
May provides in-service education to pharmacy, nursing, medical residents and fellows, and physicians on medications.
May be present for planned bedside procedures, to provide stat medications, as needed.
Provides direct consult with providers regarding patients in covered areas.
Performs drug monitoring of patients and may make appropriate changes to orders per policy. May participate in department initiatives and projects as assigned by managers.
Have legal responsibility for proper interpretation of prescriptions and the dispensing of prescribed products, and call to physician's attention any synergistic, antagonistic, and cumulative effects, as well as questionable overdoses or incompatibilities.
Specify proper storage, protection, of therapeutic agents, in accordance with the standards of the United States Pharmacopoeia (U.S.P.) and the manufacturer package insert.
Keep continually abreast of new developments in the fast-moving drug field and be aware of emerging patterns, systems, and techniques in pharmacy practice in their sub-specialty.
May contribute to the accredited teaching programs, and training courses. Participates in the training or preceptorship and education of pharmacists, pharmacy residents, interns, technicians, and pharmacy students.
Maintains records, documents patient data, prepares written reports, and provides oral reports concerning patient care.
Monitors patient medication therapy to ensure safe and effective therapy.
Provides drug information to physician, nurses and other health care professionals as related to medication therapy.
May serve and participate in departmental and attend on interdisciplinary committees.
Supervises pharmacy technicians.
Sterile and non-sterile product preparation, checking and dispensing.
Management and control of controlled substances.
Primary responder to emergency events requiring a licensed pharmacist (Rapid Response, Code Blue Resuscitation).
Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.
Education: Doctor of Pharmacy degree from a School of Pharmacy accredited by the American Council on Pharmaceutical Education.
Experience: Completion of a PGY-2 Residency program, OR completion of a PGY-1 Pharmacy Residency Pediatric program OR PGY-1 and 1 year of related hospital experience OR Five (5) years of related hospital experience.
Licensure/ Certification: Licensed as a Pharmacist by the California Board of Pharmacy.

Current American Heart Association (AHA) Certification for Basic Life Support (BLS) for Healthcare Providers -OR- Current American Heart Association (AHA) Certification for Pediatric Advanced Life Support (PALS) for Healthcare Providers.
Knowledge of prescription and non-prescription drugs and drug products and their indications, efficacy, dosage, mechanisms of action; fate and disposition (if known), major contraindications and potential side effects.
Knowledge of Medical terminology and abbreviations; pharmacology and pharmacological nomenclature; medical therapeutics, pharmacokinetics, and disease states.
Knowledge of methods and procedures involved in proper handling and storing of pharmaceutical supplies.
Knowledge of influence of patient's age, sex, concomitant disease states, concurrent drug therapy, foods and diagnostic procedures on drug activity and disposition.
Knowledge of physiology, anatomy, pathology, chemistry and mathematics as they relate to pharmacy practice.
Knowledge of local, state and federal regulatory requirement related to the functional area.
Ability to apply judgment and make informed decisions.
Ability to plan, organize, prioritize, work independently and meet deadlines.
Ability to foster effective working relationships and build consensus with other departments and external vendors.
Ability to communicate effectively with others and exercise a high degree of tact in coordinating with and advising other members of hospital staff, patients, and various other professional contacts.
Maintain familiarity and operational knowledge of the departmental information and technical systems.
Ability to work with devices, information systems, automated and technical equipment utilized in pharmacy.

Compensation is based on the level and requirements of the role. Salary within our ranges may also be determined by your education, experience, knowledge, skills, location, and abilities, as required by the role, as well as internal equity and alignment with market data. Typically, new team members join at the minimum to mid salary range.
Minimum to Midpoint Range (1.0 FTE): $196,518.40 to $213,231.20.
