Job Title: Client Care Coordinator – Home Care

Job Description:
We are seeking a dedicated and compassionate Client Care Coordinator to join our team in providing high-quality home care services. The ideal candidate will serve as a liaison between our clients, their families, our team of caregivers, and client's other healthcare professionals, ensuring that each client’s care plan is tailored to their needs, consistently followed, and delivered with excellence.
Key Responsibilities:
Client Intake & Assessment:
- Conduct initial client assessments, gathering necessary medical, personal, and care-related information.
- Develop individualized care plans based on client needs, preferences, and medical requirements.
- Collaborate with healthcare providers, family members, and the care team to create the best care plan.
Care Plan Management:
- Monitor and adjust care plans as needed based on client progress or changes in health conditions.
- Ensure the care plan is updated regularly to reflect the client’s evolving needs.
- Communicate any changes in care plans to clients, family members, and caregivers.
Scheduling & Coordination:
- Schedule and coordinate caregiver shifts to ensure continuous, high-quality care.
- Maintain accurate schedules and ensure that caregivers are appropriately matched with clients based on the care plan.
- Address client and caregiver concerns related to scheduling or care delivery.
Client and Family Communication:
- Serve as the main point of contact for clients and their families, providing ongoing support and addressing any concerns.
- Regularly check in with clients and families to ensure satisfaction with care services.
- Educate clients and families on the caregiving process and services offered.
Caregiver Support & Supervision:
- Provide guidance and supervision to caregivers, ensuring they adhere to care protocols and deliver care with compassion.
- Assist with caregiver performance evaluations and provide feedback or additional training as needed.
- Support caregivers by addressing any concerns or challenges they face on the job.
- Provide backup support to cover caregiver shifts in time sensitive situations when a caregiver calls out and a replacement has not been found in time
Documentation & Compliance:
- Maintain accurate and up-to-date client records, including care plans, progress reports, and service logs.
- Ensure that all documentation complies with relevant regulations, industry standards, and company policies.
- Assist with audits and compliance checks to ensure adherence to healthcare regulations and home care guidelines.
Problem Solving & Conflict Resolution:
- Address and resolve any issues or conflicts that arise between clients, families, and caregivers.
- Investigate complaints or concerns and implement solutions to improve care and client satisfaction.
Requirements:
- Previous experience in a home care or healthcare setting is highly preferred.
- Strong organizational skills and the ability to multitask in a fast-paced environment.
- Excellent verbal and written communication skills.
- Ability to develop and maintain positive relationships with clients, families, and caregivers.
- Knowledge of home care regulations and best practices.
- Proficiency in Microsoft Office Suite and other relevant software tools.
- Must be compassionate, empathetic, and patient.
- Ability to work both independently and as part of a team.
- Flexibility to handle a dynamic schedule and the ability to problem-solve effectively.
- Must be knowledgeable of Medicaid Aged and Disabled, Indiana Pathways and Health & Wellness (Indiana State Waiver Programs)
Preferred Qualifications:
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification (a plus).
- Previous experience as a Care Coordinator or in a similar role.
- Knowledge of Medicare, Medicaid, and insurance processes (a plus).
